1. • • In-network insurance patients should also choose the 1st Visit-Consultation & Exam option. We will adjust the visit type within the system at the time of appointment.

2. Set up an account or use an existing account (Gmail, Facebook etc.)

3. Choose a "Date & Time."

4. You will receive an email confirmation and a link to provide your history & information. Be sure to fill it out! (8-12mins)

• Please fill out the forms as soon as possible so you don't forget.
