- If you are new to the clinic, please choose the "1st Visit - Consultation & Exam" option at the top so we can get to know you.
In-network insurance patients should also choose the 1st Visit-Consultation & Exam option. We will adjust the visit type within the system at the time of appointment.
- Set up an account or use an existing account (Gmail, Facebook etc.)
- Choose a "Date & Time."
- You will receive an email confirmation and a link to provide your history & information. Be sure to fill it out! (8-12mins)
If you prefer handwritten forms you can Download the forms (here) and let us know you will be bringing them in.
Please fill out the forms as soon as possible so you don't forget.
